Advance Care Team
We now have an Advance Care Team Member to support our Provider in an effort to deliver effective patient-centered care. We have assigned Roxan as a Medical Assistant care coordinator who is responsible for our patients’ care.
She works under protocol to triage patients, refill meds, perform routine health maintenance and chronic disease monitoring tests, answer and return phone calls from patients. She coaches patients about action plans and facilitates referrals. This has been working really well for all of us.
